Twenty years have passed, Rambo lives in Thailand, where he earns his living with a rather peaceful profession: he catches snakes and, having a small boat, takes tourists around the local area. But one day his peace was disturbed. A group of American missionaries from one of the Christian churches asked John to take them to neighboring Burma, where a brutal civil war was going on.

The missionaries wanted to deliver humanitarian aid to a village where the Karen inhabitants were suffering from the oppression of the Burmese government troops. Rambo tries to dissuade the volunteers, firstly, the trip to Burma is very dangerous. Secondly, John did not believe that this way anything could be changed. But the young and pretty Sarah Miller manages to convince the former soldier.

Rambo delivers the missionaries to Burma, despite the fact that they are attacked by local pirates on the way. However, upon returning home, John learns that something bad has happened to the volunteers: they have been captured by the Burmese. The pastor of the church from which the missionaries went to Burma hires a detachment of former military special forces, who agree to rescue the prisoners.

In this operation, Rambo is assigned the role of a guide and boatman. He must simply deliver the mercenaries to the place and wait for them with the result. But Rambo was not brought up to sit in the rear, because he is a man of war, an ideal killing machine, he fought all his life not for the country, but for himself.

Having come to such a disappointing conclusion, Rambo decides to get involved in this matter, although he had recently promised himself not to take an active part in adventures to save the world and all progressive humanity...
